撰文时，新闻报道近日接受两次肝移植手术的女病人情况仍然危殆，更指另一医院早前看漏该女病人的病历而开漏药，可能导致病人肝病病发。

报章用"A blunder by doctors led to the acute liver failure of a mother who later needed two transplant to save her life." 来做标题，指医生犯错令一位妈妈出现急性肝衰竭，其后需要接受两次肝脏移植手术以保性命。

"blunder"作名词用，解大错。

大家还记得已故的「九龙皇帝」吗？笔者上周读报知道他生前于观塘的一些墨宝被政府的外判商以白色油漆盖掉，而又有人在白色油漆上再涂鸦。有意见认为若政府树立一个告示板或张贴告示，让大家留意到曾灶财街头涂鸦的文化价值，这个错误是可以避免的（"The blunder might have been avoided had the government put up a signboard or notices drawing people's attention to the cultural value of Tsang's work."）

笔者希望犯错者会认真检讨，避免重犯，皆因有些错误是无法补救的。
